Tailored Care Management is a service provided to North Carolina Medicaid enrollees.  A dedicated care management team, led by a Care Manager, provides support to people who are living with mental illness, substance use disorders, or intellectual and developmental disabilities.  Our Care Management team works closely with our Medicaid members to develop a care plan designed to address their specific needs.  The plan brings together all of the people who care for you:  your doctors, therapist, family members, and the community resources that help you live your best life.

What Tailored Care Management Provides:​

• ​A dedicated Care Manager who will listen to your needs and coordinate care between all of your providers and your team members.

• Personalized and specialized support that empowers people living with mental illness, substance use disorders, and intellectual and developmental disabilities.

• Support to address additional needs such as housing, employment, food insecurity, physical health, finances, and safety, which may affect overall health and well-being.
